North Central London COVID-19 Booster and Flu vaccines survey

Please complete this short survey about the flu and COVID booster vaccines. The information you provide will help shape our work on these programmes. It should take no more than five minutes to complete and your responses are completely anonymous.

Please do not enter any personal information in any questions; if you do it will be removed before sending for analysis. Howver, if you provide an email address you will be sent a receipt and a link to a PDF copy of your response.
