Overview

We would like to hear your views about our practice changing the Primary Care Network (PCN). This would mean the practice leaving the Central One Islington Primary Care Network which currently consists of seven practices and joining N2 Primary Care Network. The new PCN will consist of eight practices.

The reason for changing the Primary Care Network to another service is to work with other practices who share the same vision of patient care as ourselves and work to tailor the provision of care to our population needs.

Our GP practice will not close or that you will need to find a new doctor.  Our GP practice services will continue uninterrupted.

What are Primary Care Networks (PCNs)?

To meet patient needs, GP practices are working together with community, mental health, social care, pharmacy, hospital and voluntary services in their local areas in groups of practices known as primary care networks (PCNs).

Primary Care Network are led by clinical directors who may be a GP, general practice nurse, clinical pharmacist or other clinical profession working in general practice. They build on existing primary care services and enable greater provision of proactive, personalised care for patients and more integrated working with other health and social care for people close to their homes.

The services you may see provided by your Primary Care Network are additional appointments being provided during 6.30-8.00pm and consultations with a Pharmacist, Dietitian, Physician Associate, Physiotherapist. 

Please take the time to feed your thoughts back about this proposed change of Primary Care Network and patient experience of accessing services at our GP practice. 

The results of the patient survey will be published on the practice website and shared with the Patient Participation Group (PPG).
