Long COVID self declaration

Overview

A national service to support general practice in managing patients with Long COVID.

The service allowed for the identification, assessment, referral and support for patients with:

• • Ongoing COVID-19 symptoms from 4 to 12 weeks after infection; and
  • Post COVID-19 symptoms that continues for more than 12 weeks without any alternative diagnosis.

You are to complete this form and submit to commissioners in order to receive the final  25% of payment for the Long COVID Enhanced Service by 31 March 2022
