Dear Patient

We would like to hear your views about the proposed merger between Islington Central Medical Centre & Roman Way Medical Centre and what impact it may have on you. We must emphasise that neither practice will be closing as a result of this merger.

This short survey has been developed and sent to patients at your practice.

Please take the time to feed back your thoughts about this proposed merger.  

We will study all the responses, and this will inform our application for the practice merger. The results will be published on our websites after the engagement.

Alternatively, paper versions of the survey are available at the practice if you would prefer to complete it in this way.
