2025-26 SDF PCN Development Funding - Progress and Evaluation Report
Overview
Action required: PCNs are asked to complete the following online form by Friday, 8th May 2026.
As part of 2025/26 SDF PCN Development funding, PCNs were asked to select one or both of the following themes to deliver with allocated development funding:
Theme: Workforce (supporting skills and capabilities)
- Funding a PCN Workforce Education Lead - separate from NCL Training Hub funded Workforce Education Leads.
- Support MDT working and ARRS supervision.
- Funding a PCN Quality Improvement Lead.
Theme: Improving ways of working
- Reducing unwarranted variation and increasing operational efficiency across the PCN.
- Adopting / embedding technology to support at-scale working across the PCN.
- Service and/or network development through PCN collaboration with network practices and VCFSE / local system partners.
- Addressing health inequalities through PCN development and collaboration with network practices and VCSFE / local system partners.
PCNs are now asked to complete an online progress and evaluation report for the purpose of:
- Providing assurance that funds have been spent in line with national and local guidance.
- Providing assurance of delivery in line with the PCN plan and local guidance.
- Collecting examples of best practice that can be shared between PCNs.
- Collating learning to inform our approach for how we implement similar funding in future years.
Action required: PCNs are asked to complete the following online form by Friday, 8th May 2026.
PCNs are asked to consider this impact utilising SMART (Specific, Measurable, Achievable, Relevant, Time-bound) methodology. Please only complete a progress and evaluation report for the theme(s) which your PCN has worked on.

Please note, this webpage includes information regarding 'Developing neighbourhood health service models' - this is not part of this reporting cycle. PCNs are asked to evaluate progress against only the themes (workforce and/or improving ways of working) delivered.
