NHS North Central London ICB - The Hillview Surgery Patient Survey

Overview

This survey relates to the retirement of Dr S Samuel, The Hillview Surgery, 114 Finchley Lane, Hendon NW4 1DG.

After many years of providing healthcare services to the local community, Dr Saramma Samuel of The Hillview Surgery, has notified NHS North Central London Integrated Care Board (NCL ICB) that she wishes to retire and resign from her contract and would like to close the practice on 31 May 2023.  NCL ICB would like to thank Dr Samuel for her work and dedication to the practice and patients. This however means that the premises will no longer be available for future services from 31 May and the ICB is required to support patients to find new practices to register with.   

You do not need to do anything now as you will remain registered with The Hillview Surgery. You will still be able to see a GP, and request prescriptions or referrals.      

We have identified that there are 41 practices locally up to three miles from the surgery. There are nine practices within one mile and 27 practices within two miles. Details of the practices in the surrounding area have been provided in a letter sent to all patients. All patients identified as vulnerable will be contacted and supported to register at a neighbouring practice by the ICB and the staff at The Hillview Surgery.

We are also writing to all the neighbouring practices to ensure that they are able to register you when the practice closes on 31 May 2023.

We would like to seek your views on the closure of the practice, and finding and registering at a new practice.
