2024-25 SDF PCN Development Funding – Evaluation Report
Overview
As part of 2024/25 SDF PCN Development funding, PCNs were asked to select one or more of the following themes to deliver with allocated development funding:

Theme 1: Workforce / supporting skills and capabilities

Funding a PCN Workforce Education Lead - *separate from NCL Training Hub funded workforce education leads.

Support MDT working and ARRS supervision.

Funding a PCN Quality Improvement Lead.

- Theme 2: Improving ways of working
- Reducing unwarranted variation and increasing operational efficiency.
- Using technology to support at-scale working across the PCN.
- Theme 3: Other areas of work
- PCNs may choose to allocate funding to other areas of work that support the development and collaboration of the PCN and in line with the objectives of the PCN DES.
PCNs are now asked to submit an online delivery evaluation report for the purpose of:
•Providing assurance that funds have been spent as in line with national and local guidance.
•Collecting examples of best practice that can be shared between PCNs.
•Collating learning to inform our approach for how we implement similar funding in future years.
PCNs are asked to complete this online evaluation report considering SMART (Specific, Measurable, Achievable, Relevant, Time-bound) methodology by Monday, 2nd June 2025. Please only complete an evaluation for the theme(s) which your PCN has been worked on.
PCNs were also requested to complete the Support Level Framework (SLF) tool in discussion with member practices with the output being an action plan that PCNs will own, deliver and build on. PCNs have indicated this discussion has now taken place and as such meetings are being scheduled with NCL ICB Primary Care/Borough Integration Commissioners to reflect on the SLF tool and action plan during Q1 25/26.

PCNs are asked to evaluate only the themes (workforce / improving ways of working / other areas) delivered as outlined at the top of this webpage.
