Overview

 

Camden CCG are starting a new service in October of this year for people in Camden who have been diagnosed with high blood pressure (hypertension), yet despite medication still have what is known as ‘uncontrolled blood pressure (BP).

 This survey aims to evaluate how we can provide a service to best meet patient needs.

Please note that these survey responses are anonymous.

This survey will take around 20 minutes. Please feel free to only answer as many questions as you feel comfortable with.
