Survey for parents and carers about children aged 5 - 15 years receiving the COVID-19 vaccine

Overview

The NHS is now offering the COVID-19 vaccine to all children aged 5 to 15. There is a lot of information available about the vaccine and vaccination process, but we appreciate you and your child may still have questions and we would like to know what they are. We have produced this short survey to capture these.

Although we will not be able to answer your questions directly here, we will use them to shape and improve the information we provide about the COVID-19 vaccine for children, and will do our best to provide answers to your questions within the resources we produce.

Everyone completing this survey must:  

• be a Barnet, Camden, Enfield, Haringey or Islington resident 
• have parental responsibility for a child/children aged 5 to 15 years

The survey should take approximately 10 minutes to complete and your responses will be completely anonymous.

Additional information

Please do not enter any personal information; if you do it will be removed before sending for analysis. However, if you provide an email address you will be sent a receipt and a link to a PDF copy of your response. Your email address will not be retained.

The survey requires some essential cookies to operate. If cookies are not enabled you will not be able to participate. These cookies are retained for no longer than 24 hours.
