Overview

Dear Clinical Colleague,

Camden CCG is considering ways to improve the delivery of care and support for people with one or more long term condition (LTC).  The conditions under review include diabetes, COPD, atrial fibrillation, heart failure, hypertension, stroke and chronic kidney disease. This consultation seeks to find out what might help people living in Camden to manage their conditions with greater confidence and what support General Practice requires to deliver this care most effectively.

From your perspective as a clinician I want to know what you have found useful, what services are working well and where improvements could be made? Knowing what these are will help the CCG plan the best services for the future.

Thinking about your patients with one or more long term conditions, I would like you to complete this survey to help me understand how you manage your patients, what problems or barriers you may encounter in delivery of care and what you think would help you and your patients.

The short survey in the link below is open until 6pm on Friday 15th December and it would be great if you could complete so we can incorporate your views in proposals to take forward in future discussions.
