Islington INC Evaluation Proposal
Overview
Dear colleague,
The Integrated Networks Co-ordination (locally called INC meetings) started in 2015. For seven years, they have been bringing together Islington’s community health and social care organisations to plan ongoing care for complex residents. Islington’s Better Care Fund provides £1.4m per year to deliver the INC service.
- There are 8 multi disciplinary teams that meet weekly or fortnightly, working at a Primary Care Network level.
- Since INC started, we have had significant developments which have had varying levels of impact on the service including the following:
- Multiple waves of COVID (different health and care models, different needs, shielded patients)
- National (Primary Care Networks, Ageing Well, Social Prescribing)
- Local (Fairer Together, digital developments)Working towards Children’s INC models
- Growing membership; housing, voluntary sector, acute trusts
- Across the Covid pandemic the INC service was stood down while teams where re-deployed.
- We think this is a sensible time to conduct a light-touch review of where the INC service is now and how they are developing.
The review will focus on the following 6 key priorities
- Current process, function and partnership roles
- Membership and partnership engagement
- Measurement of outcomes
- Areas of strength
- Areas of opportunity
- Strategic Direction
Our partners play a key role not only in how the model runs and the impact on our patient’s lives.
We therefore wanted to ensure we have given all involved the opportunity to make observations about how the model currently runs, and key areas where we could potentially improve, in order to shape the future INC model.
